Mine's the exact same, old Intellivision cart/same instructions wording and all. They were just on a smaller piece of paper though, and I think without the year too.

Everything included: cartridge, overlay, box, and manual. Although the last page of the manual has been torn and is missing. Doesn't affect gameplay. Other than that, it's in mint condition! Let me know if you have any questions.
