I thought it looked a bit familiar, so I checked my shelves, and I have a copy of it as well. As the auction notes, it's dubbed "The Final Vectrex Cartridge", and you choose games by setting the dip switches and rotating the yellow dial. My copy is "boxed" with two sheets of paper showing all the positions you need for each game.
I suspect I got it from David Elfstrom when he sold his Vectrex collection, as I think I got a couple of the other very early multicarts (such as the one Richard H made) at the same time, and the Vectorcade.

I have no idea who made it, there are no credits in the jumper settings manual sheets, and the circuit board appears to be a modified repro board (with copyright 2003 on the bottom).
